Select the application for download

Select the desired download location on your computer

Navigate to your selected download location double click on the file to open

You should have something similar to this, note that fields highlighted in red are required fields

Fill out the form, at any point this form can be saved and closed for completion at a later time.

Some fields on the form are drop down menus that can be scrolled through.

Please ensure all the drop down menus are appropriately selected

To sign the form first click on the signature field

You will be prompted to create a new digital ID

If you have already created a digital ID it will automatically load and this field will not be seen

Use the default setting of a new digital ID file

Fill out the information as prompted

Establish a password,this will be used for any other future digital signatures

Enter Your established password as required

Click on SIGN

You will be prompted to save the file one last time

Your document is now signed and authenticated

Click Submit to initiate the sending process

You will be given the option to send via Outlook  or your preferred web-mail account

If using outlook a new message window will be loaded and the form attached automatically

You may add any message or attach additional files to this E-mail  such as engineering drawings

and insurance certificates

First you must choose your web-mail service

Next you must provide your E-mail service address

Some E-mail services will show a security notification

Allow access or accept when prompted
